Parentesco, estructura familiar y carga percibida en cuidadores de Alzheimer.

RESUMEN

[ABSTRACT] During the lasts years, many estudies focused on the caregivers of Alzheimer patients are being carried out. In this sense, one of the most important aspect is refered to the way in which the patient?s cares are distribuited in their home, together with others caregivers?s implication living or not with the patient. Most of these patiens are being cared by their daughters who are been considerated a part of a population wich requires a special attention.

OBJECTIVE: the purpose of the present study is to describe the relation that can be established between the perceived burden by Alzheimer caregiver and some variables such as : - Number of caregivers in charge of the care of the patient - Number of persons who lives with the patient - Family-relation between the caregiver and the patient.

METHOD: Ninety main caregivers of Alzheiemer patient participated in this study, taking as such those in which we can find the main responsability of the patient´s cares. These were evaluated by The Burden Scales by Zarit (1980) and a Brief Questionaire about Alzheimer caregiver´s personal and familiar aspects.

RESULTS: Results show the differences which were statistically significative in the perceived burden between daughters and no daughters (p=0.025), between living only with the patient or living with more people (p=0.000) Nevertheless, these diferences do not appear between only one caregiver or several caregivers. In this interaction with these tree variables, the groups which showed more burden were those in which the daughters were the main caregiver, who live with other relatives apart from the patient, even if they share or not the care.

DISCUSSION: A possible explication to all this can be found in the wide variety of roles in which these caregivers have to develop such as a caregivers, a mothers, a wife, a worker... In this way, they have to share their time between all these roles, while the others caregivers whose role is only the care of the Alzheimer´s patient have not to.Introducción: En los últimos años se están llevando a cabo numerosos estudios centrados en los cuidadores de Enfermos de Alzheimer. Uno de los aspectos más importantes en este sentido corresponde a como se distribuyen los cuidados del enfermo en el hogar, así como a la implicación de otros cuidadores, vivan o no con el enfermo. La mayoría de estos enfermos son cuidados por sus hijas, convirtiéndose estas así en una población que merece una especial atención.

Objetivo: El objetivo del presente trabajo es indagar en las relaciones que pueden establecerse entre la carga percibida por el cuidador de un enfermo de Alzheimer, y variables tales como: - Número de cuidadores encargados de atender al enfermo. - Número de personas que conviven con el enfermo. - Relación de parentesco entre cuidador y enfermo.

Metodología: En esta investigación participaron 90 cuidadores principales de enfermos de Alzheimer, entendiendo como tales aquellos en los que recae la principal responsabilidad en el cuidado del enfermo. Los enfermos mostraban distintos grados de deterioro, desde leve hasta severo. Las medidas administradas fueron las siguientes: 1- Escala de Carga de Zarit, 1.980 2- Breve Cuestionario sobre aspectos personales y familiares del cuidador

Resultados: Los resultados obtenidos muestran diferencias estadísticamente significativas en carga percibida entre hijas/no hijas (p= 0,025), y entre convivir sólo con el enfermo/convivir con más personas (p=0,000), no apareciendo estas diferencias entre cuidador único/varios cuidadores. En la interacción de las tres variables, los grupos que más carga mostraron fueron los de hija cuidadora principal, que convive con otros familiares además del enfermo, independientemente de si compartían cuidados o no.

Discusión: Una posible explicación a lo anterior puede venir dada de la sobrecarga de roles en que se ven inmersos estos cuidadores (cuidadora, madre, esposa, trabajo fuera de casa, ...). De este modo, todo su tiempo se repartiría entre estos ámbitos, frente a aquellos otros cuidadores en los que el cuidado del enfermo de Alzheimer ocupa prácticamente todo su tiempo.
